Merchandise Not Received Statement

    I am fully aware that the shipping company’s (UPS or FEDEX) website shows that this item was delivered to the address that I provided to Quality Cables USA when I placed my order online with one of its websites.
    I am requesting a FULL REFUND of my funds as I am not in possession of these products nor am I aware of anyone receiving this shipment and or the merchandise, I.E.: “services have not been rendered”.
    All shipments are insured and will be investigated by the Shipper/Local Authorities Etc.
    I hereby declare this statement to be true.
